1. What information do we collect, and how do we collect it?

When you register for an account through our Site through “Gmed – Modernzing Medicine”; our patient portal provider (“Patient Portal”), we collect Personal Information, such as your name, date of birth, billing and address, and other contact information. You may use the Patient Portal to share information about your health, including your height, weight, health records, lab results, and other health related information with us. This data is used to populate a profile within your account. You may also choose to use the secure messaging feature in the Patient Portal, which will allow the exchange of communications between you and our medical professionals. These communications may contain additional Personal Information.
Appointments. When you make an appointment with us, we may collect information to further assist you in making this appoint. This may include: your first name, last name, date of birth, gender, email address, and mailing address, or phone number. You may also be asked questions regarding your current medications, height, and weight, as well as other information about the reasons for your appointment.
Payment Data. Your payments will be made through our third-party payment processor to help us facilitate payment. We have contracted with such payment processors to ensure they protect and limit the use of your Personal Information. Dr. Olivera does not receive or directly store your credit card, debit card or other financial information when you make a payment- such information is provided directly to our payment processor. However, GO Gastro does receive information about your payments and your contact information (e.g., name, email address, mailing address, phone number). GO Gastro will keep a record of your payments.
Data Automatically Collected
Information collected through automated methods include information from cookies, web beacons, pixel tags, clear gifs, and other tracking technologies (collectively “Tracking Technologies”) on our Site. The Tracking Technologies on our Site may collect information such as:

GO Gastroenterology is the sole and exclusive owner of such de-identified and aggregated information, including if Olivera de-identifies Personal Information so that it no longer considered Personal Information under applicable laws.
7. Children’s Information.
The Site and our services are not directed at children under 18 years of age. We do not knowingly collect, sell, use, or share Personal Information from children under 18. If we obtain actual knowledge that any Personal Information we collect has been provided by a child under the age of 18 for which we do not have parental consent, we will promptly delete that information. If a parent or legal guardian learns that their child provided us with Personal Information without his or her consent, please contact us and we will make commercially reasonable attempts to delete such Personal Information.
