1. What information do we collect, and how do we collect it?

Personal Information” is information that can be linked or reasonably linked to you as an identified individual or an identifiable individual. The information we may collect from and about you falls into three categories: (a) information you provide to us, (b) information we collect through automated methods when you use our Site, and (c) from other sources (as further detailed below).

Personal Information We Collect Directly From You 

You are not required to actively provide Personal Information to visit the Site. However, Personal Information is required to use certain features of our Site, for example, to create an online account, contact us via our contact form (where we may retain your message’s content and our response), or make appointments. In such circumstances, we may be unable to provide you with our Services unless you provide your Personal Information.

Patient Portal Accounts. When you register for an account through our Site through “Gmed – Modernzing Medicine”; our patient portal provider (“Patient Portal”), we collect Personal Information, such as your name, date of birth, billing and address, and other contact information. You may use the Patient Portal to share information about your health, including your height, weight, health records, lab results, and other health related information with us. This data is used to populate a profile within your account.  You may also choose to use the secure messaging feature in the Patient Portal, which will allow the exchange of communications between you and our medical professionals. These communications may contain additional Personal Information.  

Appointments. When you make an appointment with us, we may collect information to further assist you in making this appoint. This may include: your first name, last name, date of birth, gender, email address, and mailing address, or phone number. You may also be asked questions regarding your current medications, height, and weight, as well as other information about the reasons for your appointment. 

Payment Data. Your payments will be made through our third-party payment processor to help us facilitate payment. We have contracted with such payment processors to ensure they protect and limit the use of your Personal Information. Dr. Olivera does not receive or directly store your credit card, debit card or other financial information when you make a payment- such information is provided directly to our payment processor. However, GO Gastro does receive information about your payments and your contact information (e.g., name, email address, mailing address, phone number). GO Gastro will keep a record of your payments.

Data Automatically Collected

Information collected through automated methods include information from cookies, web beacons, pixel tags, clear gifs, and other tracking technologies (collectively “Tracking Technologies”) on our Site. The Tracking Technologies on our Site may collect information such as: 

Most browsers automatically accept Tracking Technologies, but you can reset your browser to refuse Tracking Technologies. Please note that you may not be able to access the full functionality of our Site if certain Tracking Technologies are disabled on your browser. We may obtain data about individuals from various third-party companies and public sources, and we may combine that data with other Personal Information. This enhances our existing data about our users, improves our ability to contact you, and enhances our marketing capabilities.

Professional Advisors. We may disclose Personal Information with our professional advisors, such as our attorneys, accountants, financial advisors, and business advisors, in their capacity as advisors to GO Gastroenterology Ltd.

Other. We may disclose Personal Information with third parties when explicitly requested by or consented to by you, or for the purposes for which you disclosed the Personal Information to us as indicated at the time and point of the disclosure (or as was obvious at the time and point of disclosure).

Change in Ownership. If all or part of GO Gastroenterology Ltd and/or Ghazanfari and Olivera is sold, merged, or otherwise transferred to another entity, we may transfer Personal Information to such entity as part of that transaction.

Government Officials/Law Enforcement. We may disclose Personal Information to the proper authorities if we become subject to a subpoena or court order or if we are otherwise legally required to disclose such information. We may also use and disclose information about you to establish or exercise our legal rights, to enforce the Terms and Conditions or this Privacy Policy, to assert and defend against legal claims, or if we believe such disclosure is necessary to investigate, prevent, or take other action regarding actual or suspected illegal or fraudulent activities or potential threats to  any person or entity.

4. Social Media

We are active on social media, including Facebook and Instagram (“Social Media”). Anything you post on Social Media is public information and will not be treated confidentially. We may post (or re-post) on the Site and our Social Media pages any comments or content that you post on Social Media platforms. 

The Site allows you to connect and share data with Social Media platforms. These features may require us to use Automated Technologies, plug-ins, and APIs provided by such Social Media platforms to facilitate those communications and features. The Site may use advertising networks and services offered by Social Media platforms to deliver advertising content. Use of these services requires Social Media platforms to implement Automated Technologies or pixel tags to deliver ads to you while you access the Site.

Your use of Social Media is governed by the privacy policies and terms of the providers that own and operate those websites and not by this Policy. We encourage you to review those policies and terms.

5. Data Retention 

We may retain and store Personal Information for as long as needed for the purposes indicated in this Privacy Policy, which may be indefinitely. 

6. Use and Disclosure of Non-Personal Information

We may collect, use, disclose, transfer, and otherwise process de-identified and aggregated information that it receives or creates for any purposes in its sole discretion, in compliance with applicable laws. GO Gastroenterology is the sole and exclusive owner of such de-identified and aggregated information, including if Olivera de-identifies Personal Information so that it no longer considered Personal Information under applicable laws.  

7. Children’s Information

The Site and our services are not directed at children under 18 years of age. We do not knowingly collect, sell, use, or share Personal Information from children under 18. If we obtain actual knowledge that any Personal Information we collect has been provided by a child under the age of 18 for which we do not have parental consent, we will promptly delete that information. If a parent or legal guardian learns that their child provided us with Personal Information without his or her consent, please contact us and we will make commercially reasonable attempts to delete such Personal Information.

8. How Do We Protect Information Collected About You?

GO Gastro uses commercially reasonable security measures to protect Personal Information. If a breach of your Personal Information occurs, we will notify you of the breach if required under applicable law.
